Peripheral neuropathy

    Disorganization of motor function in two extremities resulting in extreme limitation despite ≥3 months treatment.

    In plain terms: Despite treatment, has peripheral neuropathy left you with severe weakness, numbness, or loss of sensation in two limbs that limits walking, balance, or hand use?

    Marked limitation in physical functioning AND one mental area despite ≥3 months treatment.

    In plain terms: Has the neuropathy significantly affected both your body and your mind — pain affecting concentration, sleep, mood?
