    Disorganization of motor function in two extremities, despite adherence to treatment for ≥3 months.

    In plain terms: Despite Parkinson's treatment, do you have severe movement problems in two limbs — tremor, rigidity, or balance trouble — that limit standing or walking or using your arms?

    Marked limitation in physical functioning AND in one mental area despite ≥3 months treatment.

    In plain terms: Despite treatment, has Parkinson's significantly affected both your body and your mind — memory, attention, mood?

    Evidence required:RFC (residual functional capacity) assessment
