To be approved under this listing, a claim must meet any one of the criteria below — and the medical records have to prove it.

    Resting ankle-brachial index (ABI) ≤0.50; OR resting toe systolic pressure ≤30 mm Hg; OR specific Doppler/imaging findings of significant peripheral occlusive disease.
