Ischemic heart disease

To be approved under this listing, a claim must meet any one of the criteria below — and the medical records have to prove it.

  1. A

    Sign- or symptom-limited exercise tolerance test demonstrating specific ischemic findings (ST-segment changes, drop in BP, ventricular arrhythmia) at workload ≤5 METs.

    In plain terms: Have you had a stress test that had to be stopped early because of heart symptoms or abnormal findings?

  2. B

    Three separate ischemic episodes requiring revascularization (angioplasty, stent, bypass) within consecutive 12-month period.

    In plain terms: In the past year, have you had three or more procedures to open or bypass blocked heart arteries?

  3. C

    Documented coronary artery disease with ≥50% narrowing of nonbypassed left main, ≥70% narrowing of another nonbypassed major coronary artery, or ≥50% narrowing of bypassed coronary artery, AND inability to perform an exercise tolerance test at workload >5 METs.

    In plain terms: Has a cardiac catheterization shown major artery blockages, and have you been unable to do a stress test at a normal workload?
