    Three pathologic fractures within a 12-month period.

    In plain terms: Have you had three or more bone fractures in the past year that happened from minor injuries — or with no injury at all?

    Inability to ambulate effectively, OR inability to use upper extremities for fine and gross movement effectively, lasting or expected to last 12 months.

    In plain terms: Have the fractures left you unable to walk well, or unable to use your hands and arms normally for at least a year?
