    eGFR ≤20 mL/min/1.73m² OR serum creatinine ≥4.0 mg/dL on at least 2 evaluations ≥90 days apart within 12 months.

    One of: renal osteodystrophy with severe bone pain and imaging abnormalities; peripheral neuropathy with severe motor disturbance; persistent fluid overload; anorexia with recent weight loss.
